Hypertension Program

Hands of Compassions’ hypertension program provides thorough patient care for the diagnosis and treatment of difficult hypertension disorders. Services are provided by a multidisciplinary team including nurses and therapists highly experienced in the care of retirees.

Studies have shown that medication alone is not enough to control hypertension. Patients must follow a healthy diet as recommended by their doctor, and after a lifetime of different habits, this can be hard to maintain. Weight control may also be an issue, and a handout may not be enough to help patients. Multiple visits and training sessions have been shown to improve understanding and lifestyle habits.

When complications are likely, Hands of Compassions’ nurses provide skilled monitoring and medication adherence. Once a patient’s condition has stabilized, our nurses teach patients and caregivers how to monitor their blood pressure readings.

Medication Management Program

You must know what medications you can’t take with others, for example: what do you do when you have a cold? Can you take cold medicine with your other medications? And what does that red pill do?

Managing all of this can be tricky, and medication inconsistency ranks as a serious problem for millions of adults post hospital discharge. It is important for you to take your medication as prescribed. Hands of Compassions’ medication management program teaches you about medications and how to regulate them safely. We access, clarify, and review medications for a portion of each skilled home health visit. Our over-arching goal is to prevent negative reactions, ensure consistency, and identify potential problems before they happen.

When adding telehealth to the usual care plan, it creates an additional protective layer compared to the usual care alone. Home health patients receiving telehealth monitoring show greater improvements in overall health.

Hospital-grade equipment placed in the home helps us continue care between visits. This equipment captures real-time data that is viewed by our team 7 days a week. To ensure 24/7 care, our equipment has cellular functionality for when land lines are not available. Information such as weight, oxygen saturation, peak flow, blood glucose level and more are transmitted to our team throughout the day.
